Healthwatch England urges NHS to improve discharge processes

Healthwatch England found that 51% of patients are not given details of post-discharge support, risking readmission. 59% were not asked if they needed transport home, breaching guidelines. Patients reported feeling neglected and confused. One woman had to travel several hours home alone after sustaining injuries. Another, Gladys, received inadequate post-discharge support. Healthwatch urges the NHS to improve discharge processes before winter pressures. The Department of Health and Social Care has been approached for a response.
