University graduate dies in hospital after receiving disconnected oxygen mask

A young woman died in hospital after being given a disconnected oxygen mask and left in a corridor. Clarissa Street, 24, was admitted with dizziness and unwellness, but spent an hour in a corridor with the non-functional mask before her condition worsened. An inquest heard that paramedics initially thought she was having a panic attack. She later died from a pulmonary embolism.
