Inquest hears gardaí have no recollection of inquiries into missing man who died by suicide

An inquest has heard that two gardaí who responded to a search for Seán O'Neill, a man who died by suicide after leaving a hospital emergency department, have no recollection of the incident. No details of their initial inquiries were logged on the Garda Pulse system. The 25-year-old had been arrested and transferred to Connolly Hospital Blanchardstown in March 2017 after attempting suicide twice while in custody. Officers Michael McGloin and Andrew Reynolds were dispatched after he left the hospital, but could not recall viewing CCTV or searching the grounds. His body was found a day later a short distance from the emergency department. The inquest also heard a security officer asked O'Neill to return three times, but he refused. A consultant said there were no legal grounds to physically detain him. Neither garda was interviewed during the initial investigation into the death.
